Terms of Use
Effective date: October 22, 2026
These terms apply to your use of meadowbloomneuro.com, a website run by Meadowbloom Center for Pediatric Neurology ("we", "us"). By using the website, you agree to them.
General information only
Nothing on this website is medical advice, a diagnosis or a treatment plan for your child. Please read our Medical Disclaimer.
In an emergency, call 911
If you think your child is having a medical emergency, call 911 or go to the nearest emergency room. Do not use this website or email to ask for urgent help.
Using the website does not make your child our patient
A doctor-patient relationship begins only when Dr. Shah sees your child at an appointment we have confirmed with you. Signing up for updates, downloading a form or emailing us does not book or confirm an appointment.
Forms
You may download and print the forms on our Patient forms page to prepare for a visit. The Seizure Action Plan belongs to the Epilepsy Foundation, and the Vanderbilt ADHD rating scales belong to the National Institute for Children's Health Quality (NICHQ). We provide those copies for your family's personal use, and their owners' terms apply to them.
Our content
The text, logo, photographs and design of this website belong to Meadowbloom Center for Pediatric Neurology or are used with permission. You may view and print pages for your own personal use. You may not copy, change or republish them for any other purpose without our written permission.
Links to other websites
We link to outside organizations because families find them useful. We do not control those websites and are not responsible for their content, accuracy or privacy practices. A link is not an endorsement.
Acceptable use
Please do not try to break, overload or gain unauthorized access to the website, and do not submit false information or someone else's details through the signup form.
Accuracy
We try to keep the website accurate and current, but we do not promise that every page is complete, accurate or up to date. Information may change without notice.
No warranty
The website is provided "as is" and "as available". To the fullest extent the law allows, we disclaim all warranties, express or implied, about the website and its content.
Limitation of liability
To the fullest extent the law allows, we are not liable for any loss or damage that results from using, or being unable to use, this website or the websites it links to. This does not limit any liability that cannot be limited by law, and it does not affect the care we provide to patients.
Privacy
Our Privacy Policy explains what the website collects. Our Notice of Privacy Practices explains how we protect patients' health information.
Changes to these terms
We may update these terms. The new version applies from the effective date shown above.
